A single-screen subjective, objective, assessment, and plan (SOAP) is a method of documentation healthcare providers (HCPs) use to write notes for a patient’s chart—providing a framework for evaluating information in an organized manner.

This new intuitive user interface enables clinicians to review a patient’s medical history, document findings and plans, and edit all on a single screen.


The intended result: simplifying their workflow and reducing the need for clicking and scrolling between sections on the EHR to document a patient’s clinical visit, according to ModMed.

Yes! The single-screen is accessible via an iPad, or desktop web browser.

The EMA is also available on a smartphone through ModMed’s PocketEMA available in Apple’s App Store or Google Play for Android users.
